During your Home Medicines Review, a credentialed pharmacist will take the time to understand your health and how you use your medicines. The review may cover:
- A thorough check of all your prescription and over-the-counter medications
- Prescriptions
- Checking that dosages and timing are right for you
- Helping you understand what each medication does and why you take it
- Up to two follow-up visits if clinically indicated
After the review, your pharmacist provides a detailed report to your GP with recommendations to improve your medication management.
