A Home Medicines Review (HMR) is a government-funded service where a credentialed pharmacist visits you at home to review all your medications. The goal is to ensure your medicines are working safely and effectively, identify any potential problems, and provide recommendations to your GP.

Anyone in Australia with a Medicare card or DVA card who has a referral from their GP or eligible specialist can access our Home Medicines Review service. For our script filling and delivery service, anyone with a valid eScript can place an order.

No. Home Medicines Reviews conducted through MedtoDoor are funded by the Australian Government. There is no cost to you.

Yes, for a Home Medicines Review you need a referral from your GP, specialist physician, specialist psychiatrist, specialist in pain medicine, or specialist in palliative medicine. Your GP can write this referral if they believe you would benefit from a medication review.

For script delivery, you don’t need a referral — just a valid eScript.

A credentialed pharmacist visits your home at a time that suits you. They will review all your medications — prescription, over-the-counter and supplements — and discuss how you use them. The review typically takes 30–60 minutes. After the visit, the pharmacist prepares a report with recommendations for your GP.

Generally, you can have a Home Medicines Review once every 24 months. However, if your GP determines there has been a significant change in your condition or medications, more frequent reviews may be arranged.

Yes. If clinically indicated, up to two follow-up visits can be conducted. The first follow-up occurs between 1 and 9 months after your initial review, and the second at least 1 month after the first (within 9 months of the initial review). Follow-ups focus on resolving medication issues identified during your initial review.

Yes, the review is conducted in your home to allow the pharmacist to see all your medications, including those stored in different locations. In limited circumstances (cultural or safety reasons), an alternative location may be approved with prior authorisation.
