How the partnership works

We set you up.
You receive a unique pharmacy partner page on the MedtoDoor website, plus a printed QR code to display in-store.

2Identify
You identify eligible patients.
When a patient presents with GP referral for Home medication Review, you introduce them to MedtoDoor’s HMR service.

3Book
The patient books through your page.
The patient scans your QR code or visits your unique MedtoDoor page and books their review. Your pharmacy is automatically recorded as the referring partner.

4Deliver
We deliver the service.
A MedtoDoor credentialed pharmacist conducts the Home Medicines Review in the patient’s home and provides the report to their GP.

What your patients need
To be eligible, your patients need:
- 1 A current Medicare card or DVA card
- 2 A referral from their GP or eligible specialist
- 3 To live in a community setting (not hospital or aged care facility)
If the patient doesn’t yet have a referral, encourage them to speak with their GP.

Important information
- The Home Medicines Review is conducted by a MedtoDoor credentialed pharmacist, not your pharmacy staff.
- The HMR report is sent to the patient’s GP.
- There is no cost to the patient — the service is funded by the Australian Government.
